District Nursing: In this first theme, you will gain an in depth and applied knowledge and understanding of the various approaches that are used to care for those being supported by district nursing teams, range of common presentations and care needs, and through this, demonstrate a systematic knowledge of underlying aetiology of these presenting complaints. This will involve demonstrating competence in assessing, planning, implementing and evaluating the care of those being supported by these services, such as those who are housebound and requiring regular district nursing care, such as acute and chronic illness management, wound dressings, catheter care, vaccinations and medicines management.
Primary Care Nursing: In this second theme, you will gain an in depth and applied knowledge and understanding of the various approaches that are used to care for those accessing primary care, such as GP surgeries, and through this, demonstrate a systematic knowledge of underlying aetiology of these presenting complaints. This will involve demonstrating competence in assessing, planning, implementing and evaluating the care of those being supported by these services, such as those who are housebound and requiring regular district nursing care, such as acute and chronic illness management, wound dressings, catheter care, vaccinations and medicines management.
Community Matron: In this third theme, you will gain an in depth and applied knowledge and understanding of the various approaches that are used to care for those being supported by community matrons, and through this, demonstrate a systematic knowledge of underlying aetiology of the long-term conditions and complex circumstances that those receiving care from community matrons often have. This will involve demonstrating competence in assessing, planning, implementing and evaluating the care of those being supported by community matrons, such as those with chronic and complicated health conditions.
Specialist Community Nursing: In this forth theme, you will gain an in depth and applied knowledge and understanding of the various approaches that are used to care for those being supported by specialist community nursing teams, such as preventing dependency, early supported discharge, and frailty teams. This will involve demonstrating competence in assessing, planning, implementing and evaluating the care of those being supported by a variety of specialist teams, such as those with chronic and complicated health conditions.
Working within teams to meet the needs of those accessing primary and community care: In this final theme, students will have the opportunity to pull together the knowledge from across the module, specifically in relation to managing those in primary and community settings. Students will also be introduced to the role of teamwork in the management of patients and the impact of this on decision making, standardized communication and escalation techniques, by exploring a range of case studies and opportunities to practice in simulation.
